Proof that you have completed the CAA Med 160 form online- a screenshot of payment is fine- Cellma continues to be unreliable and if it is not working, this is the only way I can undertake a medical.
Corrective lenses and glasses. If you need glasses, please bring your prescription from the optician. Class 1 pilots who require corrective lenses/glasses will need an optician’s report every 2 years. Please ensure your optician has completed the CAA Med 162 eye test form and bring it to your medical.
Copies of any medical reports relating to declared conditions. If you are a new patient to my practice, it is helpful to bring old reports. If you have seen me before then I only need reports on any changes.
Up to date prescription for any medication that you routinely take.
Your logbook or a record of your total flight hours and hours flown since last medical.
